MOV is Apple’s QuickTime container and MP4 is essentially its ISO-standardised descendant. Both can wrap H.264 or HEVC video plus AAC audio, which is why the two formats look interchangeable. The practical difference is container metadata: MOV files may carry ProRes, timecode tracks, alpha channels or multiple audio tracks that MP4 players do not expect.

The video stream is dropped before decoding ever happens, so the pixels in your MOV are never processed. Only the audio is decoded and re-encoded to MP3, which is why a large video file converts in seconds rather than minutes and why the output is a few megabytes instead of a few hundred.
